Each new school year we ask if you can fill out our re-enrollment application. Just in case you moved, got a new phone number, have another friend or family member that you would like to authorize to pick-up your child(ren). We will also need a Health Evaluation for your child since it is a new school year. 

Once filled out, please drop form to Prime Time's office at 1215 W. Summit St. on or before Monday, August 24, 2020.
