Welcome new families! Please take a moment to print out the registration form and the parent handbook. The parent handbook is for you to keep. It has all of our information in there including about us, our policies, hours of operation etc. Once completed the registration form is filled out, please bring it to our physical address at 1215 W. Summit Street. 

*Along with the enrollment application we will need a copy of your child's Health Appraisal and Immunization records. Or if you do not immunize your child, we will need a copy of a Non-Medical Exemption Form.
